44 year old male patient Nguyen Van Th. in Hanoi diagnosed with sinusitis for many years. Recently, he experienced an increase in airway discomfort, the situation of acute sinusitis continued in several consecutive periods.

The patient came to the Central Otolaryngology Hospital, through a physical examination and a subclinical appointment, the doctor detected polyps on both sides of the patient’s sinusitis. Doctors have performed polyp removal surgery with the help of new instruments with many advantages.

 

The surgery of removing sinus polyps for Th patient was televised from the operating room to the hall of the Central Otolaryngology Hospital for experts to discuss and exchange expertise.

One day after the surgery, the surgical site does not bleed, Th. patient is recovering well and is expected to be discharged in the next few days.

This is one of two surgeries that Assoc. Siow Jin Keat - Senior Advisor of Department of Otorhinolaryngology, Tan Tock Seng Hospital Singapore and Assoc.Prof.Dr. Tran Viet Luan - Head of Department of Otorhinolaryngology, Medical University, Pham Ngoc Thach medical hospital performed on polyp sinusitis and was live broadcast from the operating room to the hall of the Central Otolaryngology Hospital where seminars "Advanced Endoscopic sinus surgery" and a training course for nurses on "High-speed drilling equipment and hand curettage for endoscopic sinus surgery" were taking place on September 24.

This surgery applied FESS method with unique and new techniques, drawn from the practical experience of the two doctors. The process uses multifunctional grating drilling system IPC, Microdebrider and Fusion locator of Medtronic, which helped to increase the accuracy and minimize the surgery time as well as the recovery time of the patient.

According to Assoc.Prof. Pham Tuan Canh - Director of Central Otolaryngology Hospital, before the sinus surgery, the surgeons used tweezers to clamp on polyps, or into the organs of the sinus nose to remove, however the lesions were wide. Nevertheless, with the new tool - the IPC Powered Instrument, Microdebrider, which are referred to a "both cutting and absorbing tool", they help cut the polyps neatly, reduce bleeding and damage of the mucosa.

"This means with this device, the surgeon will only the exact part that is needed to remove, to shorten the surgery time, thus invade patients less and improve recovering speed after surgery." - Assoc.Prof. Pham Tuan Canh said.

Also, this new device has the support from Medtronic’s image diagnosis and Fusion locator so wherever the device goes, it notifies the surgeon if there is about to be a "collision" or other healthy organizations are affected. As a result, surgery safety is increased.

“Locator system helps find sinus cells in the nooks and crannies, but only removes the inflamed ones, leaving behind healthy mucosa. Then it is this mucosa that covers and heals the niches so the healing time will be shortened”- Director Pham Tuan Canh added.

Also, according to Assoc.Prof. Pham Tuan Canh, the nature of this new system of tools such as handpiece, is inserted to support surgery to be faster and more accurate. In fact, in the past, a surgery used to last 2-3 hours, but when applying this device, the time decreased by about 1.2 to 2/3 compared to the surgeríes using old equipment.

Season changing, air pollution: An increase of 30% of patients suffering sinusitis

According to Assoc.Prof. Pham Tuan Canh - Director of the Central Otolaryngology Hospital, the humid tropical climate and environmental pollution are always biggest causes of otolaryngology problems, including both internal and external treatment.

Although there have been no studies on the number of patients coming to the Central Otolaryngology Hospital due to environmental pollution, recently, the number of patients has increased. Currently, the hospital has an average of 1,000 patients a day, of which patients with sinusitis have tremendously increased, accounting for about 30% of the total number of patients visiting the Central Otolaryngology Hospital. Among those, many patients are small children since sinusitis is associated with tonsillitis infections.

“Seasonal changes lead to an increase in illnesses regarding ear, nose and throat; especially for patients with atopic allergies, smoke is also a factor to this increase. Assoc.Prof. Pham Tuan Canh said that the treatment of sinusitis using Eastern medicine and herbal medicine also needs specific studies. In case of acute sinusitis, it is necessary to prioritize Western medicine treatment to avoid complication.

For the use of Eastern and traditional medicine, people need to select products of clear origin, at licensed facilities.

For sinusitis, priority is given to medical treatment over surgery.

Sinusitis can cause complications into the eye socket, into the brain ...
